to Obtain Quotations for Life, Disability and/or Dental Insurance
Life
Programs
- We administer employee and dependent basic life plans,
employee and dependent optional life plans and accidental
death, dismemberment and personal loss programs under this
benefit category.
To
prepare a quote for you, we need a description of your current
plan(s), your carrier(s) and the rates you are being billed.
In addition, we need a census of your group including each
employee's sex, date of birth, annual salary and occupation
(not required if your benefits are not related to salary
or position) or amount of coverage.
Disability
Programs
- We
administer short-term disability plans, long-term disability
plans, NY-DBL plans and enriched NY-DBL plans under this
benefit category.
To
prepare a quote for you, we need the same kind of information
for these plans as detailed above for life plans. Your census
must include covered employees' annual salaries and occupations.
Dental
Programs
- Our
plans include variations covering all dental services and
orthodontia coverage for children and adults.
To
prepare a quote for you, we need the same kind of information
about your dental plans as above for life and disability plans
— and also let us know if your plan is contributory.
Your dental census should include each eligible employee's
sex and dependent status (single, employee plus spouse, employee
plus child(ren) or employee plus family).
